The following information is provided to make things easier for you and us. We are excited to work with you towards your goal of better health.
Please bring any of the following to your initial visit:
Health Insurance card
New patient paperwork (if done in advance)
X–Rays, MRI or other reports if you have any
Shorts or Yoga style pants
Filling out the New Patient Paperwork (Links Below) prior to the first appointment will help to make sure we stay on schedule and allow us to spend your valuable time getting to the root cause of your health issue. If you are unable to print/fill out the paperwork, please arrive 15 minutes early.
Please plan up to 60 minutes for the first appointment.
